Help need with adding new templates

Hello All

I'm attempting to add new templates to a website I'm building.

But i can not figure out the following to get login to work correctly.

Here is the New login section on the new Template.



And here is the login section before changing the with new Table




Can someone please guide or help me please to make the changes to the new template so i can get the login to work correctly please.
